Once downloaded, please print / complete the form and bring it in to our Sayre offices, or mail it in to the address show above. Please do not try to email it.

Providing the information and completing the "E-Business Information" form is the first step in registering a cooperative member to use NFEC’s new online bill pay system. Once NFEC has received this required information, it will be added to our system’s records so that the online bill pay system will recognize the member and their account. This will allow the member to set-up a user name and password so that they can both view and pay their bill on our website.

Signing this form means that the member understands the request for this information. Northfork Electric will not sell or give this information to any other vendors or solicitors.
